I. Basic Information
Name: Ms. Song
Gender: Female
Age: 53
Chief Complaint: Fractured and mobile left mandibular tooth; requesting extraction and implant‑supported restoration.
Past Medical History: Denies any systemic diseases, and no known food or drug allergies.
Present Dental History: A left mandibular tooth has fractured and become mobile, affecting mastication; presented to our clinic for consultation.
II. Clinical Examination
Intraoral examination was performed.

Radiographic examination was performed.

III. Diagnosis and Treatment Plan
Diagnosis: The left mandibular posterior tooth had been previously treated and is now fractured, with chronic periapical inflammation, granuloma, and a buccal purulent sinus.
Treatment Plan: Minimally invasive extraction of the involved tooth, followed by a restoration‑driven approach using one Hanzu® implant (size 4.5 × 13 mm).
IV. Treatment Procedure
Surgical Procedure: Under local anaesthesia, the tooth was extracted, the osteotomy was prepared, bone graft material was placed, and the Hanzu® implant was inserted. The wound was sutured.

Prosthetic Phase: In April 2025, the definitive restoration was delivered.

V. Case Summary
This case involved immediate implant placement into an extraction socket in a patient with osteoporosis, infection, and significant alveolar bone loss and resorption. The Hanzu® implant system demonstrated strong self‑tapping capability, leading to a favourable clinical outcome.
Compared with delayed implant placement on the right side, the immediate placement on the left side maximised the preservation of alveolar ridge contour and volume.
