Case Report: Full-Arch Immediate Implant Placement and Immediate Loading
I. Basic Information
Name: Mr. Liang
Gender: Male
Age: 65 years
Chief Complaint: Most teeth missing in both arches; remaining teeth have been mobile for several years.
Past Medical History: No significant findings.
Present Dental History: The patient reported that in recent years, teeth have either exfoliated spontaneously or been extracted elsewhere due to mobility. The remaining teeth are loose and displaced, affecting mastication; presented for prosthetic rehabilitation.
II. Clinical Examination
Specialised Examination (Intraoral and Extraoral):
Frontal view: Facial asymmetry noted, with reduced lower facial height.
Profile view: Convex facial profile.
Intraoral examination: Teeth #11, #12, #16, #17, #18, #21, #22, #23, #24, #26, #27, #28, #31, #35, #36, #37, #38, #42, #44, #45, #46, #47, #48 were missing. The alveolar ridge in the edentulous areas showed moderate contour. The remaining teeth exhibited gingival recession and Grade II–III mobility.
Temporomandibular joint examination: Normal mouth opening and opening path; no clicking or tenderness in the joint areas.

Auxiliary Examination:

Radiographs revealed moderate alveolar bone atrophy in the edentulous areas, and varying degrees of bone resorption around all remaining teeth.
III. Diagnosis and Treatment Plan
Diagnosis:
Maxillary and mandibular partial edentulism;
Chronic periodontitis.
Treatment Plan:
Extraction of all remaining teeth;
Immediate loading with full‑arch implant‑supported prostheses in both arches;
Submerged (healing‑abutment) placement at site #36.
IV. Treatment Procedure
Surgical Phase (January 6, 2025): Under local anaesthesia with articaine, all remaining teeth were extracted. An incision was made, alveolar bone was contoured, osteotomy sites were marked, and osteotomies were sequentially prepared to the predetermined depths. A total of 12 Hanzu® implants (6 in the maxilla and 6 in the mandible) were placed. Multifunctional abutments were connected, and the wounds were sutured. Transfer copings were attached, impressions were taken, silicone bite registrations were made, and provisional prostheses were delivered immediately.


Sutures were removed 2 weeks later. At 3 months post‑operatively, final impressions were taken for fabrication of the definitive prostheses.
Final Restoration (May 26, 2025): Definitive prostheses were delivered.


Pre‑operative and post‑operative comparisons were documented.
Regular follow‑up visits were scheduled.
V. Case Summary
This case involved immediate implant placement and immediate loading for full‑arch restorations in both jaws. The Hanzu implant system features a mountain‑shaped double‑screw design and a neck‑constriction design, which provide excellent primary stability, making it particularly suitable for immediate placement and immediate loading protocols.
