I. Basic Information

Age: 72
Chief Complaint: Missing right maxillary teeth for many years; requesting restoration.
Past Medical History: Denies hypertension, cardiovascular disease, or diabetes; no significant family history; self‑reports no history of hepatitis or other infectious diseases.
Present Dental History: Previous implant placement in the left mandible; recently unable to chew on the right side; presented for consultation.
II. Clinical Examination
Specialised Examination: Teeth #15, #16, and #17 were missing; alveolar ridge was low and flat; no signs of maxillary sinusitis; poor overall oral hygiene.
Intraoral Photographs: Taken.

Pre‑operative CBCT: Performed.


III. Diagnosis and Treatment Plan
Diagnosis: Partial edentulism with severe bone defect.
Treatment Plan: Implant‑supported restoration.
Detailed Plan: External sinus floor elevation with bone grafting and simultaneous implant placement.
IV. Treatment Procedure
Surgery Date: September 12, 2024, 4:00 PM.
External sinus floor elevation was performed in the right maxillary premolar and molar regions, with simultaneous placement of two Hanzu® implants.
Pre‑operative planning was completed.

Surgical procedure was carried out as planned.
