Case Report: Bone Augmentation with Immediate Implant Placement and Restoration in the Left Mandibular Molar Region

I. Basic Information

Name: Ms. Gao

Gender: Female

Age: 60

Chief Complaint: Missing left mandibular teeth with frequent dislodgement of the removable partial denture; requesting implant‑supported restoration.

Past Medical History: Denies any systemic diseases, and no known food or drug allergies.

Present Dental History: The patient received a removable partial denture ten years ago; poor retention affecting mastication; presented to our clinic for consultation.


II. Clinical Examination

Intraoral examination was performed.

III. Diagnosis and Treatment Plan

Diagnosis: Missing left mandibular first and second premolars, and first and second molars, with insufficient alveolar ridge width.

Treatment Plan: Restoration‑driven approach using two Hanzu® implants (size 3.5 × 10 mm) for the premolar sites and two Hanzu® implants (size 4.0 × 10 mm) for the molar sites, with buccal bone augmentation.

 

IV. Treatment Procedure

Under local anaesthesia, after disinfection and incision, autogenous bone was harvested during osteotomy preparation. The Hanzu® implants were placed. In the left mandibular molar region, tent screws were placed on the buccal aspect of the two implant sites to create space for bone grafting, followed by graft placement and wound suturing.

Follow‑up (Prosthetic Phase – January 2025): The definitive prostheses were delivered. Multifunctional abutments were connected, and crowns were placed. Due to insufficient occlusal space, the posterior abutments were left exposed after crown placement.

V. Case Summary

The Hanzu® implant system demonstrates excellent osseointegration. The sloped shoulder design allows for more favourable bone coverage over the implant, which is conducive to bone attachment and growth, thereby ensuring long‑term implant stability and facilitating favourable stress distribution.
