Case Report: Internal Sinus Lift with Immediate Extraction and Implant Placement for Maxilla

I. Basic Information

Name: Mr. Deng

Gender: Male

Age: 62 years

Chief Complaint: Multiple maxillary teeth missing for several months, affecting dietary intake.

Past Medical History: Previously healthy.

Present Dental History: Multiple maxillary teeth missing for several months, affecting mastication; presented for restoration.


II. Clinical Examination

Specialised Examination:

Frontal view: Largely symmetrical, with reduced lower facial height.

Profile view: Straight facial profile.

Intraoral examination: Teeth #18, #15, #14, #13, #12, #11, #21, #22, #23, #24, #25, #26, #27, and #28 were missing; alveolar ridge contour was moderate; no gingival erythema or swelling.

TMJ examination: Normal mouth opening and opening path; no clicking or tenderness in the joint areas.

Auxiliary Examination: Radiographic examination was performed.


III. Diagnosis and Treatment Plan

Diagnosis: Maxillary partial edentulism.

Treatment Plan: Maxillary implant placement with immediate restoration.

IV. Treatment Procedure

 

Surgical Phase (March 2025): Routine disinfection and draping were performed. After local anaesthesia, an incision was made, a full‑thickness flap was reflected, granulation tissue was debrided, and alveolar bone was contoured. Five Hanzu® implants were placed; internal sinus floor elevation was performed at site #26. Cover screws were connected to the implants at #26, while multifunctional abutments were connected to the remaining implants. The gingiva was closed with tight sutures. Impressions were taken, occlusion was recorded, and a provisional prosthesis was fabricated.

Follow‑up: Sutures were removed approximately 1–2 weeks later according to the patient's condition.

 

Final Restoration: The definitive restoration was completed at 3 months post‑operatively.

V. Case Summary

This case involved immediate implant placement and immediate loading for a maxillary full‑arch restoration. The S‑shaped double‑screw design of the Hanzu® implant system makes it highly suitable for immediate loading protocols.
