Case Report: Immediate Implant Placement with Labial GBR Vertical Bone Augmentation for Two Maxillary Central Incisors and the Right Maxillary Second Premolar
I. Basic Information

Chief Complaint: Mobility of the upper anterior teeth and missing right maxillary teeth; seeking implant‑supported restoration.
Past Medical History: Denies any systemic diseases, and no known food or drug allergies.
Present Dental History: The patient received porcelain‑fused‑to‑metal fixed prostheses on the anterior teeth ten years ago; due to poor retention affecting mastication, he presented to our clinic for consultation.
II. Clinical Examination
Intraoral examination was performed.

Radiographic examination was performed.

III. Diagnosis and Treatment Plan
Diagnosis: Missing left maxillary central incisor and right maxillary second premolar; the right maxillary central incisor exhibits Grade III mobility, with labial alveolar bone loss and gingival erythema.
Treatment Plan: Immediate extraction and implantation of the right maxillary central incisor, with labial bone grafting (GBR). For the left maxillary central incisor and right maxillary second premolar, a restoration‑driven approach will be adopted using two Hanzu® implants (size 4.0×13 mm) and one Hanzu® implant (size 4.0×10 mm).
IV. Treatment Procedure
Surgical Procedure: Under local anaesthesia, an incision was made at the right maxillary second premolar site, osteotomy preparation was performed, and one implant (4.0×10 mm) was placed. For the anterior region, the incision was made for debridement; due to labial alveolar bone loss at the right maxillary central incisor site, autogenous bone was harvested during osteotomy preparation, and then mixed with bone graft material and collagen membrane, followed by placement of one implant (4.0×13 mm).


Follow‑up (Prosthetic Phase – January 2025): A provisional crown was fabricated, impressions were taken, a transfer coping was used for implant‑level impression, and the definitive prosthesis was delivered.


V. Case Summary
The Hanzu® implant system demonstrates excellent osseointegration. The sloped shoulder design allows for more favourable bone coverage over the implant, which is conducive to bone attachment and growth, thereby ensuring long‑term implant stability and facilitating favourable stress distribution.
